Assessment of agreement between manual sphygmomanometers and automatic devices for blood pressure measurement

Abstract

Introduction: The accuracy of blood pressure measurements with manual sphygmomanometers can be influenced by several factors.

Objective: To assess the concordance between manual sphygmomanometers and automatic devices in the measurement of systolic and diastolic blood pressure in adult patients in Ecuador.

Methods: A total of 133 adult patients from Ecuador were analysed. Metrics such as intraclass correlation coefficient (ICC), Bland-Altman analysis and linear regression were used to assess concordance in blood pressure measurement. Cronbach's alpha coefficient was calculated to verify internal consistency and reliability of measurements.

Results: There was strong agreement between blood pressure measurement methods. Intraclass correlation coefficients showed high values: 0.970 and 0.986 respectively for single measurements, and even higher, 0.985 and 0.993 respectively for average measurements. F-tests showed significant differences between the methods for both measurements. In addition, Cronbach's alpha coefficients indicated strong internal consistency: 0.985 for systolic and 0.993 for diastolic blood pressure. These findings suggested discrepancies between the methods, but despite the differences detected, the results showed adequate agreement for practical application.

Conclusions: Strong agreement between measurement methods was evident, supported by high ICC values and Bland-Altman analysis. Although no significant linear association was found in the regression analysis, the consistency in measurements suggests a practical similarity between the two methods for measuring blood pressure.
