Evaluation of the effectiveness of mammography for the early detection of cancer in women in Ecuador

Abstract

Introduction: Screening plays a crucial role in women with a high risk of breast cancer.

Objective: To evaluate whether mammography is effective for the early detection of breast cancer in women aged 40-70 years over a four-year period, in patients at a clinic in the city of Santo Domingo in Ecuador, during the period 2018-2022.

Methods: The study was analytical, observational, longitudinal and prospective. A total of 166 women were studied. The McNemar statistical test was applied. Results: Approximately 43 % had a family history of breast cancer. After 2 years, 41.6 % with no initial abnormalities maintained that status, while 34.3 % of those who showed abnormalities at baseline no longer had them. However, 24.1 % with initial abnormalities maintained them at 2 years. After 4 years, 38.6 % without initial abnormalities continued without them, but 3 % developed signs of cancer. Of the group with initial anomalies, 53 % no longer showed them, although 5.4 % persisted with them, confirming the risk. In total, 8.4 % had persistent abnormalities, suggesting possible breast cancer at the 4-year follow-up. The bilateral exact significance value in the two controls was <0.001 on McNemar's test.

Conclusions: There is statistical evidence to affirm that there is a significant difference between the results of baseline and 2-year and 4-year follow-up mammograms in terms of detection of abnormalities or indications of breast cancer.
