Endo-periodontal lesions, their treatment, and prognosis

Abstract

Endo-periodontal lesions represent a complex condition involving both the dental pulp and the periodontium, and their treatment requires an integrated approach. The objective was to interpret endo-periodontal lesions, their treatment, and prognosis through a literature review. A narrative review and an observational and retrospective study were conducted, including publications in English and Spanish between January 2010 and December 2023. The information search was carried out in databases such as PubMed, Scopus, Web of Science, Google Scholar, and university digital libraries. Eighteen relevant studies were selected that provided data on diagnoses, treatment techniques, and clinical outcomes. The findings revealed that the interrelationship between the dental pulp and the periodontium is fundamental to understanding the etiology and management of these lesions. Periodontal regeneration through innovative techniques, including the use of growth factors and biomaterials, shows promising results in the recovery of dental support structures. The precise diagnosis of endo-periodontal lesions is based on a combination of medical history, clinical and radiographic evaluations, and pulp vitality tests. Successful treatments generally require addressing the endodontic part first and then the periodontal part. Evidence suggests that proper management can result in high success rates and significantly improve the long-term prognosis. In conclusion, the treatment of endo-periodontal lesions demands a multidisciplinary approach and a deep understanding of the connections between pulp and periodontal tissues.
