Knowledge and acceptance of cervical cancer screening using the Papanicolaou test in pregnant women

Introduction: Pregnancy may offer an opportunity to capture women overdue for cervical screening, but knowledge and acceptance of the Papanicolaou test during gestation are variable.
Objective: To synthesize the evidence on knowledge and acceptance of cervical cancer screening using the Papanicolaou test in pregnant women, as well as the associated barriers and facilitators and the role of prenatal care as an opportunity for screening uptake.
Methods: Mixed-methods systematic review with narrative synthesis, without meta-analysis. Primary studies published in Spanish or English between January 2020 and April 2026 were searched in PubMed/MEDLINE, Scopus, Web of Science, and LILACS. Pregnant women with data on knowledge, acceptance, willingness, Pap test uptake, or related barriers and facilitators were included. Two reviewers performed study selection and data extraction by consensus, with third-reviewer arbitration when needed.
Results: Nineteen studies were included. Evidence showed heterogeneous knowledge and acceptance. Greater knowledge and previous screening were associated with a more favorable disposition, whereas fear of harming the pregnancy, discomfort, cost, and preference for the postpartum period limited acceptance. Variability in recommendations, provider- and system-level factors, and incomplete postpartum follow-up were also identified.
Conclusions: Prenatal care may be a useful entry point for Pap screening, but its value depends on consistent counseling, uniform implementation, and continuity of care. Overall certainty was limited by conceptual heterogeneity and the predominance of observational studies.
