Abstract

Diabetes mellitus (DM) and periodontal disease (PD) exhibit a bidirectional relationship in which both conditions mutually influence each other, worsening their progression. PD is one of the main complications of uncontrolled DM, while periodontal inflammation can negatively affect glycemic control. The aim of this study was to diagnose the relationship between periodontitis and diabetes in patients from the Hospital General del IESS in Ibarra, Ecuador, during the period from May to September 2023. A mixed-methods study was conducted, including a systematic review of the literature and a clinical evaluation of 75 individuals, divided into 25 diabetic patients and 50 controls. Data collection involved the use of periodontograms and surveys to identify the degree and stage of PD in both groups. The results showed that all diabetic patients presented PD, with severe forms in stage 3 being the most frequent (16 %), while only 50 % of the control group had PD, predominantly moderate forms in stage 2. Additionally, diabetic patients developed severe PD even with lower levels of dental plaque accumulation, and a higher prevalence of severe PD was observed in Black women. It is concluded that the bidirectional relationship between DM and PD is evident, with DM being a determining factor in the severity of PD. This study highlights the importance of interdisciplinary management to improve oral health and glycemic control in diabetic patients.
