Middle meningeal artery embolization versus conventional treatment for chronic subdural hematoma

Abstract

Background: Chronic subdural hematoma is a nonacute condition associated with recurrence and reintervention after conventional treatment. Middle meningeal artery embolization has been proposed as a strategy targeting the vascular component of dural neomembranes.

Objective: To assess the effectiveness and safety of middle meningeal artery embolization, alone or added to conventional treatment, compared with conventional management in adults with chronic, subacute, or nonacute subdural hematoma.

Methods: A systematic review with narrative synthesis and no meta-analysis was conducted. We searched PubMed/MEDLINE, Embase, Scopus, Web of Science, the Cochrane Library, and LILACS for studies published or available online between 2020 and 2025 in English or Spanish. We included randomized clinical trials, comparative cohort studies, and case-control studies. We conducted screening and eligibility assessment in duplicate; we resolved disagreements through discussion or with a third reviewer.

Results: A total of 286 records were identified, 50 full-text reports were assessed, and 15 studies were included. In several studies, embolization was associated with fewer events related to recurrence, progression, treatment failure, or the need for surgical rescue. However, the effect varied across surgical and nonsurgical settings, with differences in endovascular technique, comparators, outcomes, and follow-up.

Conclusions: Embolization may be considered a complementary or alternative strategy in selected patients, but not a general substitute for conventional treatment. Further studies are needed to better distinguish clinical settings and to standardize assessment of the clinical and radiological course.
