Abstract

Introduction: Tracheostomy is a fundamental procedure for patients requiring prolonged mechanical ventilation. The tracheal cannula induces histological changes in the mucosa, the temporal progression and precise characteristics of which require further characterization in our setting.

Objective: To characterize the histopathological changes of the tracheal mucosa and their temporal evolution in patients with tracheostomies in Camagüey.

Methods: An observational, descriptive, and cross-sectional study was conducted between February 2023 and May 2025. Twenty-three tracheal mucosal biopsies from patients aged 19 years or older were analyzed. The samples were processed using the convencional technique. Data were analyzed using descriptive statistics with SPSS Statistics 26.0.

Results: The mean age was 61.2 ± 13 years, with a predominance of the 60–79 age group and males. A defined temporal sequence was evident: congestion appeared early, ulceration was detectable from the third day onward, and metaplasia from the seventh day of intubation. Acute inflammation was present from the first few days, transitioning to a chronic pattern from the fifth or sixth day onward.

Conclusions: The most frequent histopathological changes in the tracheal mucosa after tracheostomy were congestion and acute inflammation, followed by ulceration and metaplasia. A progressive sequence of lesions was confirmed, with a clear relationship between intubation time and the severity of morphological and inflammatory alterations. The predominant demographic profile was men aged 60-79.
