Impact of ERAS Protocols and Regional Analgesia on Complex Abdominal Wall Reconstruction

Abstract

Background: Complex abdominal wall reconstruction involves high-demand perioperative procedures, with risk of postoperative pain, complications, readmission, and prolonged functional recovery. Enhanced recovery after surgery protocols and regional analgesia have been proposed to optimize these outcomes.

Objective: To examine the impact of ERAS protocols and regional analgesia in adults undergoing complex abdominal wall reconstruction.

Methods: The search was performed on December 30, 2025, in PubMed/MEDLINE, Embase, Scopus, Web of Science, Cochrane Library, and LILACS, and included studies published between 2020 and 2025 in English or Spanish on ERAS protocols, regional analgesia, or multimodal strategies in complex abdominal wall reconstruction. Screening, eligibility assessment, and data extraction were performed in duplicate; the findings were synthesized narratively.

Results: The search retrieved 184 records, and 16 studies were included. The findings showed shorter hospital stay, reduced opioid consumption, better pain control, and earlier recovery across different clinical settings. Variability was also observed in protocol components, the degree of protocol compliance, surgical approaches, and outcome measurement.

Conclusions: ERAS protocols and regional analgesia were associated with shorter hospital stay, reduced opioid consumption, and better pain control in complex abdominal wall reconstruction, although the heterogeneity and predominantly observational nature of the evidence limit the generalizability of these findings.
