Surgical management of gingival smile due to altered passive eruption

Abstract

Gingival enlargement, recognized for its aesthetic and functional impact, can be caused by various conditions, including impaired passive eruption and drug-induced enlargement. This article presents a clinical case of a 22-year-old male patient with a gummy smile, characterized by excessive exposure of the gum during smiling. After a precise diagnosis that included measurement of the gingival margin and the cement-enamel boundary, an aesthetic crown lengthening with osteotomy was chosen to correct the deformity. The procedure included local anesthesia, creation of a surgical guide, controlled incisions, gingivectomy and osteotomy, ensuring the preservation of gingival tissue. The results showed a significant reduction in gingival exposure from 5 mm to 1 mm and an improvement in dental aesthetics, aligning with ideal aesthetic proportions. Different techniques are discussed to address the gummy smile, highlighting that aesthetic crown lengthening with osteotomy is the treatment of choice due to its effectiveness and predictability. This comprehensive approach highlights the importance of a proper diagnosis and personalized treatment plan in resolving aesthetic dental problems.
