Inflammatory nexus between psoriasis and insulin resistance: recent evidence

Abstract

Objective: To evaluate, by means of a systematic review, the available evidence on the use of antidiabetic drugs in patients with psoriasis, focusing on their effects on clinical and metabolic parameters, in order to support the formulation of comprehensive therapeutic strategies adapted to the individual cardiometabolic profile.

Methods: A systematic review was performed in Web of Science, Scopus, SciELO, and PubMed, covering review articles and randomized clinical trials published from 2015 to the present, with emphasis on the relationship between psoriasis and insulin resistance as well as the principal therapeutic interventions reported.

Results: The identified studies indicate that central obesity and adipokine imbalance intensify inflammatory activity, while insulin resistance exacerbates cutaneous manifestations and increases cardiovascular risk. Agents such as metformin, pioglitazone, and GLP-1 receptor agonists not only improve glycemic homeostasis but also contribute to PASI reduction. Moreover, biological therapies retain their effectiveness even under metabolic dysfunction, and supplementation with probiotics and vitamin D shows a potential immunomodulatory impact.

Conclusions: The evidence examined supports the need for a multidisciplinary strategy that incorporates regular insulin resistance evaluation into psoriasis management, aiming to optimize clinical outcomes and mitigate related complications. More extensive prospective and comparative research is necessary to enhance clinical recommendations.
