Diagnostic utility of AIR and RIPASA scores in acute appendicitis

Abstract

Acute appendicitis is one of the most common surgical emergencies, and its accurate diagnosis remains a clinical challenge. Various diagnostic scales, such as AIR and RIPASA, have been developed to improve precision and reduce negative appendectomies. The objective of this study was to analyze the diagnostic utility of the AIR and RIPASA scales in acute appendicitis through a literature review. An exhaustive search was conducted on PubMed, selecting articles published between 2019 and 2024, which evaluated the sensitivity, specificity, and accuracy of these scales. The results indicate that the RIPASA scale shows high sensitivity and diagnostic accuracy, especially in Asian and Middle Eastern populations, surpassing the AIR scale in these contexts. However, the AIR scale demonstrated higher specificity and negative predictive value, making it useful for identifying patients who do not require immediate surgical intervention. Both scoring systems are valuable tools for the diagnosis of acute appendicitis, but their effectiveness varies depending on the clinical context and population. In conclusion, while the AIR and RIPASA scales offer advantages in the evaluation of acute appendicitis, their application should be contextualized and complemented with other diagnostic modalities to maximize their precision and clinical utility.
