Abstract

Right heart failure (RHF) represents a considerable challenge in cardiovascular medicine because of its significant impact on morbidity and mortality. Traditionally underestimated compared to left ventricular failure, RHF has gained attention due to its complexity and difficulties in its management. This review focuses on current pharmacological strategies, highlighting recent developments that seek to improve both quality of life and clinical outcomes for patients. Conventional and emerging therapies are discussed, including diuretics, endothelin receptor antagonists, phosphodiesterase-5 inhibitors, novel sodium-glucose cotransporter type 2 (SGLT2) inhibitors, and angiotensin-neprilysin receptor inhibitors (ARNI). These treatments show potential to improve right ventricular function and address complications associated with RHF, such as pulmonary hypertension. The methodology of this review involved a comprehensive literature search from 2019 to the present, using key academic databases to ensure inclusion of the most relevant and current studies. This review underscores the importance of ongoing evaluation and adaptation of therapies to emerging evidence to optimize the management of RHF.
