Nursing surveillance and care for hemodynamic instability after major gynecologic oncology surgery

Abstract

Background: Hemodynamic instability after major gynecologic oncology surgery is a dynamic clinical process related to perfusion, bleeding, fluid balance, transfusion, and early organ deterioration.

Objective: To synthesize the available evidence on nursing surveillance and care for detection, interpretation, and escalation of hemodynamic instability in women undergoing major gynecologic oncology surgery.

Methods: A systematic review without meta-analysis was conducted following the PRISMA 2020 guidelines. Studies published from January 1, 2020, to December 30, 2025, were searched in PubMed/MEDLINE, Embase, Scopus, Web of Science, Cochrane Library, and LILACS. Experimental or observational studies in Spanish or English addressing gynecologic oncology surgery, postoperative surveillance, hemodynamic deterioration, or nursing care were included. A narrative synthesis was used.

Results: Sixteen articles were included. The evidence showed heterogeneous definitions of hemodynamic instability and a lack of specific validation of early warning scores in this population. Postoperative hypotension, fluid balance, transfusion, and need for critical care support were associated with relevant clinical outcomes. Enhanced recovery protocols provided perioperative standardization, though they did not always specify nursing algorithms for hemodynamic monitoring.

Conclusions: Nursing surveillance should include monitoring of vital signs, urine output, fluid balance, bleeding, hemoglobin levels, lactate levels, transfusion requirements, and clinical escalation criteria. Operational definitions, alert thresholds, and standardized clinical monitoring protocols tailored to this population are needed.
