Endodontic management of the fourth MB2 canal in the maxillary first molar

Abstract

The treatment of the fourth mesiobuccal (MB2) canal in the maxillary first molar is a significant clinical challenge due to the difficulty in locating and treating this additional canal. The objective of the study was to optimize the treatment of the fourth MB2 canal in the maxillary first molar using advanced diagnostic and endodontic techniques, thereby improving clinical outcomes and reducing associated complications.  A clinical study was conducted on a 16-year-old male patient with dental discomfort in the right maxillary region. Infiltrative anesthesia with 2 % lidocaine was used, and a cameral opening with absolute isolation was performed. Fanta AF Blue S One rotary files were employed for biomechanical preparation, and intracanal medications of Otopren and, subsequently, calcium hydroxide with propylene glycol were applied. The localization of the fourth MB2 canal was carried out using cone-beam computed tomography (CBCT) and ultrasound. The obturation was performed using AH Plus sealer and the single cone technique. CBCT facilitated the precise localization of the MB2 canal, significantly improving clinical outcomes. Follow-up radiographs showed a reduction in the apical lesion. Clinically, it was observed that the use of intracanal medication between sessions, along with proper biomechanical preparation, allowed for effective disinfection and adequate obturation of the root canals. It is concluded that the use of advanced diagnostic and endodontic techniques, such as CBCT and ultrasound, is crucial for the successful management of the fourth MB2 canal in the maxillary first molar.
