Two-year follow-up of patients with chronic total coronary artery occlusions

Abstract

Introduction: Percutaneous treatment of chronic total coronary artery occlusions is one of the most complex scenarios for the interventional cardiologist. In cases of low complexity, angioplasty should be considered the therapy of choice, considering pharmacological treatment as a safe option. There are studies in our country on success rates, as well as clinical and angiographic predictors of failure of percutaneous treatment, however, there are no survival studies with this treatment option and those with optimal medical treatment.

Objetive: To evaluate the survival of patients with chronic total coronary artery occlusions.

Methods: An observational, analytical, cohort study was carried out to meet the objectives.

Results: We studied 175 patients with two years of follow-up, mean age of 61.2 years, with a predominance of male sex (69.1%). A total of 58.9% of the patients had other coronary arteries with significant stenosis in addition to the totally occluded artery. Of the patients who underwent percutaneous treatment, 55.9% were successful; the presence of collateral circulation had a statistically significant effect on the outcome. Readmission for angina was the adverse event most frequently observed during follow-up. No statistical differences were observed in terms of the occurrence of adverse events in terms of the treatment strategy chosen.

Conclusions: Attempted opening of the occluded artery did not reduce the risk of adverse cardiovascular events.
