Tooth extraction without discontinuation of rivaroxaban treatment

Abstract

Introduction: Oral surgery procedures in patients using oral anticoagulants reports many controversies, but in minor oral surgery procedures the treatment scheme should not be modified because of the risk of thromboembolism.

Objective: To present the case of a patient treated with rivaroxaban who underwent tooth extraction.

Clinical case: A 57-year-old female patient who came to the stomatology office to have a molar tooth extracted. She had a history of atrial fibrillation with treatment instituted with rivaroxaban that should not be modified. The clinical examination revealed a root remnant of the upper right first molar caused by advanced cavities. The extraction was performed with local anesthesia and the required instruments. Local hemostatic measures such as collagen plug, tranexamic acid and suture were used. The patient evolved for seven days, with rigorous recommendations, without bleeding episodes.

Conclusions: Simple dental extraction, without suspending rivaroxaban, is possible and safe with the use of local hemostatic resources.
